YES Applications

For the 2008-2009 school year, only current YES scholarshp recipients will be considered for renewal. The following items need to be provided:

• Completed application and documents, with a $45.00 application fee which is non refundable, cashiers check or money order.
• School transcript - 2.5 GPA or higher
• Transcripts under a 2.5 GPA will not be accepted.
• Parent’s most recent complete tax return
  • No tax extensions will be accepted
  • You must be shown as dependent on the return. If you are not listed on the tax return, you must provide a return with your name on it.

All applications and documents must be received in our office by May 1, 2008. Faxes and e-mails will not be accepted. Scholarship amounts vary each year depending on our budget.
